The Nursing Home Transition and Diversion Program (NHTDP) is a waiver that uses Medicaid funding to provide services to participants that allow them to return from a Nursing Home to the community (Transition) or remain in the community and avoid Nursing Home Placement (Diversion). The program was designed with the philosophy that individuals with disabilities and/or seniors have the same rights as others to be in control of their lives. This waiver program maintains the individual’s health and welfare through an individualized plan of care. Americare is now a Provider for the Nursing Home Transition and Diversion Program.
