    Americare is the only home health care agency in New York State certified in treating patients with a behavioral health diagnosis. At Americare, our nurses, therapists, and home health aides are trained and certified to provide specialized behavioral health services at home to these patients. We follow special protocols for patients suffering from – dementia, depression, and anxiety disorders. Social workers conduct home visits to perform psychosocial assessments and provide information about community resources such as assistance in filling a Medicaid application, food stamps, or programs for reduced utility costs such as Home Energy Assistance Program (H.E.A.P), Personal Emergency Response Systems (P.E.R. S) or other entitlement programs such as – Access-a-Ride, Meals-on-Wheels etc.

    At Americare, we know that working closely with hospitals and doctors is a vital step towards a patient’s short and long term recovery. When requested, an Americare registered nurse can come to the hospital to do a preliminary assessment of your home care needs. We will work closely with your doctor, medical professional and social worker to find the best program of care that will ensure that your transition back to your home is as smooth as possible.
